Generative AI has moved from novelty to core infrastructure in software products, from customer support bots to domain-specific assistants handling legal, medical, and financial data. But alongside the productivity gains comes a well-documented and persistent risk: hallucination. Understanding what hallucination actually is, and building systems that account for it, has become a baseline requirement for any engineer shipping LLM-powered features.
What Is Hallucination?
Hallucination is the term used when a large language model produces output that is factually incorrect, fabricated, or unsupported by any real source, while presenting that output with the same fluency and confidence as a correct answer. This might mean inventing a legal citation that doesn't exist, generating a plausible-looking but nonexistent API method, fabricating statistics, or misquoting a real document.
It's important to understand that hallucination is not a bug in the traditional sense. LLMs don't have a built-in mechanism for fact-checking their own output. They generate text by predicting the statistically most likely next token given context, not by querying a database of verified truths. When the model lacks sufficient grounding, either because the information wasn't well represented in training data or because relevant context wasn't provided at inference time, it fills the gap with plausible-sounding content. The model isn't "trying to lie": it is doing exactly what it's designed to do, extending a pattern, just without a factual anchor.
This matters because it reframes the engineering problem. You don't "fix" hallucination the way you'd fix a logic bug. You have to add an external layer of verification, because the model itself has no reliable internal signal that distinguishes a fact from a fabrication.
Mitigation Strategies
Retrieval-Augmented Generation (RAG). Instead of relying on the model's parametric memory, retrieve relevant documents from a trusted knowledge base and inject them into the prompt, then instruct the model to answer strictly from that retrieved content. This significantly reduces factual drift, though it doesn't eliminate it: poor retrieval quality or weak prompt constraints can still let the model blend retrieved facts with invented filler.
Structured output with mandatory citations. Force the model to return a schema where every claim is paired with a source reference, for example {"claim": "...", "source_id": "...", "excerpt": "..."}. You can then programmatically verify that the cited excerpt actually exists in the source document, using exact match or embedding similarity. If it doesn't match, the claim is discarded or flagged. This converts "trusting the model" into "verifying the model with code."
Self-consistency sampling. Run the same query multiple times with non-zero temperature and compare the outputs. Convergent answers suggest the model is retrieving a stable fact; divergent answers across runs are a strong signal that the model is extrapolating rather than recalling something grounded.
External fact-checking layers. Add a second verification pass, either another LLM call specifically prompted to check whether a claim is supported by the given context, or, better, deterministic validation through business rules, regex, or parsing when the output involves structured data like tax codes, account numbers, or dates. Never rely on an LLM to validate a numeric or structured fact that can be validated with plain code.
Few-shot calibration toward refusal. Include examples in the prompt where the correct answer is "I don't have that information." This helps counteract the model's default tendency to complete a pattern rather than admit a gap.
How to Know If the Model Is Hallucinating
This is the harder problem, because there's no reliable internal signal exposed by most APIs that says "I am uncertain here." Confidence in tone has no guaranteed correlation with factual accuracy. A few practical approaches:
- Token log-probabilities, when exposed by the API, can serve as a noisy proxy: low-probability tokens sometimes indicate model uncertainty, though proper nouns and numbers often show low probability even when correct.
- Cross-referencing against a deterministic source is the most reliable method available. If the model cites a rule, code, or figure, validate it programmatically against ground truth data rather than trusting the text.
- Discrepancy across repeated samples, as described above, is a strong practical heuristic for detecting ungrounded generation.
- Absence of a verifiable citation, when structured citation is enforced, is hallucination by definition rather than by suspicion.
- Textual red flags such as unusually precise, unsourced numbers or verbatim-sounding quotes in sparse-knowledge domains are a weak heuristic worth watching, but never sufficient on their own.
The Bottom Line
There is no dependable "hallucination detector" that works by inspecting model output alone. The only approach that holds up in production is architectural: treat every factually critical claim as something that must be verifiable by code against a trusted source, rather than something judged by the model's own apparent confidence. Systems built this way don't eliminate hallucination, but they contain it, catching it before it reaches the end user rather than after.
